Cardiac Rehabilitation take individuals on a personalised, structured journey with qualified instructors with the aim of improving the physical and mental health of People who have recently had a heart attack, heart surgery, or have a cardiac condition.

We currently offer Cardiac Rehab Phase 4, which acts as a  continuation of the physical activity rehabilitation program for individuals who have completed Phase 3, focussing on sustaining the benefits of regular exercise for long-term mental and physical wellbeing, alongside the opportunity to connect with a Community of like-minded People.

Cardiac Rehabilitation programme overview

 

 

Who is eligible?

We follow British Association for Cardiovascular Prevention and Rehabilitation (BACPR)  guidance and accept People with the below conditions onto the programme.

People with the following cardiac conditions:
  • Acute coronary syndrome (ACS)
  • Coronary revascularisation
  • Heart failure
  • Stable angina
  • Pre/post-implantation of cardiac defibrillators and resynchronisation devices
  • Post-heart valve repair/replacement
  • Post-heart transplantation and ventricular assist devices
  • Adult congenital heart disease (ACHD)
People with the following clinical conditions:
  • Atrial fibrillation
  • Non-obstructive coronary artery disease (NOCAD)
  • Peripheral arterial disease (PAD)
  • Spontaneous coronary artery dissection (SCAD)

We also accept Customers transitioning from NHS-commissioned Phase 3 to Phase 4 who have not completed their programme with us. They will need to provide the necessary BACPR transfer form information and undergo a corresponding instructor assessment.
