Cardiac Rehabilitation
Guidance for healthcare professionals
Cardiac Rehabilitation take individuals on a personalised, structured journey with qualified instructors with the aim of improving the physical and mental health of People who have recently had a heart attack, heart surgery, or have a cardiac condition.
We currently offer Cardiac Rehab Phase 4, which acts as a continuation of the physical activity rehabilitation program for individuals who have completed Phase 3, focussing on sustaining the benefits of regular exercise for long-term mental and physical wellbeing, alongside the opportunity to connect with a Community of like-minded People.

Initial health assessment
• Meet with the centre's Cardiac Rehabilitation Instructor
• Register onto a discounted membership, which includes the Cardiac Rehabilitation programme, alongside unlimited gym use, pool use, and group workout classes during and after the programme
• Explain how to access and use the centre
• Initial assessment and review
Programme support sessions
• Mid-programme personalised guidance and encouragement
• Overcome challenges and modify exercises
• Expert guidance from qualified instructors
• Discuss goals and techniques
• Ensure safety and effectiveness
Programme evaluation
• Questionnaires for completion in weeks 6 and 12 to track progress
• End of programme assessment questionnaire
Post-programme support
• Continue to access the centre facilities with their membership
•Various exit pathways available
Who is eligible?
We follow British Association for Cardiovascular Prevention and Rehabilitation (BACPR) guidance and accept People with the below conditions onto the programme.

We also accept Customers transitioning from NHS-commissioned Phase 3 to Phase 4 who have not completed their programme with us. They will need to provide the necessary BACPR transfer form information and undergo a corresponding instructor assessment.
